       เริ่ม ต้นปีใหม่ใครๆ ก็อยากประสบพบเจอแต่สิ่งที่ดี เรื่องสุขภาพก็เช่นกัน คงไม่มีใครอยากเจ็บไข้ได้ป่วยกันเสียตั้งแต่ต้นปี และเพื่อความอุ่นใจวิธีหนึ่งที่จะช่วยการันตีได้ว่า คุณมีสุขภาพร่างกายที่แข็งแรงสมบูรณ์ ไม่มีแนวโน้มที่โรคภัยจะถามหาก็คือ “การตรวจสุขภาพ”

       ไม่แปลกที่พอเข้าสู่ปีใหม่ หลากหลายโรงพยาบาลถึงอัดแคมเปญ โหมโฆษณาโปรโมชันโปรแกรมตรวจสุขภาพกันยกใหญ่ ซึ่งทุกวันนี้ก็มีโปรแกรมให้เลือกสรรมากมาย ไม่ว่าจะเป็นตรวจสุขภาพเบื้องต้น ตรวจโรคมะเร็งต่างๆ ตรวจเบาหวาน ตรวจหัวใจ ตรวจไปเสียแทบจะทุกอวัยวะ ซึ่งต้องยอมรับเลยว่าพฤติกรรมของผู้บริโภคอย่างเราๆ เมื่อมีโปรโมชันตรวจสุขภาพแบบครบเวอร์ และราคาที่ไม่หนักกระเป๋าจนเกินไปนัก ถือได้ว่าล่อตาล่อใจคนรักสุขภาพไม่ใช่น้อย

       แต่รู้หรือไม่ การตรวจสุขภาพแบบครบเวอร์หรือแบบเหวี่ยงแห อาจมีโทษมากกว่าประโยชน์

       เกี่ยวกับเรื่องนี้ นพ.ยศ ตีระวัฒนานนท์ หัว หน้าโครงการประเมินเทคโนโลยีและนโยบายด้านสุขภาพ (HITAP) อธิบายว่า จากการทำวิจัยเรื่อง “การตรวจคัดกรองเพื่อสุขภาพที่เหมาะสมสำหรับสังคมไทย” พบว่า การตรวจสุขภาพแบบเหวี่ยงแห ที่ไม่รวมการตรวจเพื่อยืนยันโรคหรือรักษาโรคนั้นจะก่อให้เกิดอันตรายต่อ อวัยวะอื่น เช่น การเอกซเรย์ การตรวจการทำงานของตับ ไต หรือการใช้เครื่องซีทีสแกน ซึ่งมีรังสีมากกว่าเอกซเรย์ 100 เท่า เป็นต้น โดยการตรวจแบบเหวี่ยงแหมีโอกาสจะเกิดผลบวกลวงขึ้น ซึ่งจะส่งผลต่อจิตใจผู้ป่วย และเกิดการรักษาที่ไม่จำเป็น เช่น การผ่าตัดส่องกล้องเพื่อตัดชิ้นเนื้อมาตรวจเพิ่มเติม

       ที่สำคัญโปรแกรมการตรวจสุขภาพของแต่ละโรงพยาบาลนั้น ก็ไม่ได้ต่างกันมากนัก ในแง่รายละเอียดการตรวจ ซึ่งการจากการสืบค้นข้อมูลโปรแกรมการตรวจสุขภาพของโรงพยาบาลต่างๆ โดย ASTVผู้จัดการ พบ ว่า โรงพยาบาลเอกชน เช่น รพ.รามคำแหง โปรแกรมตรวจสุขภาพสำหรับผู้หญิงวัยทำงาน ราคาอยู่ที่ 6,650 บาท จะให้บริการตรวจเลือดเพื่อดูความสมบูรณ์เม็ดเลือด ระดับน้ำตาลในเลือด ระดับไขมันคอลเลสเตอรอล ไตรกลีเซอไรด์ ระดับไขมันความหนาแน่นสูง-ความหนาแน่นต่ำ การทำงานของตับ ทั้งSGOT SGPT และ Alk Phos. การทำงานของไต ทั้ง BUN และ CREATININE ระดับยูริคในเลือด ตรวจค่ามะเร็งตับ ตรวจหาสารบ่งชี้มะเร็งในระบบทางเดินอาหาร ตรวจปัสสาวะอย่างสมบูรณ์ เอกซเรย์ปอด ตรวจคลื่นไฟฟ้าหัวใจ ตรวจหาภาวะหลอดเลือดแข็งตัวและอุดตัน ตรวจอัลตราซาวนด์ช่องท้องทั้งหมด และตรวจมะเร็งปากมดลูกด้วยวิธี Liquid Based เป็นต้น

       นอกจากนี้ ยังมีรายการตรวจเพิ่มเติมที่ต้องเสียค่าใช้จ่ายต่างหากให้ได้เลือกตรวจได้ ตามใจอีกด้วย เช่น ตรวจการทำงานของหัวใจขณะออกกำลังกาย 2,500 บาท ตรวจหาความผิดปกติของเต้านม 2,500 บาท ตรวจวัดความหนาแน่นของกระดูก 1,250 บาท เป็นต้น ขณะที่โปรแกรมตรวจสุขภาพเบาหวานราคาอยู่ที่ 3,850 บาท โปรแกรมตรวจสุขภาพหัวใจ 5,250 บาท

       รพ.เอกชนอีกแห่ง อย่าง รพ.นนทเวช ก็เริ่มมีการโฆษณาการตรวจสุขภาพแบบพื้นฐานปี 2557 โดยการตรวจBe Healthy 1&2 ราคา 3,900 - 6,900 บาท Be Active ราคา10,000-14,000 บาท ขณะที่การตรวจสุขภาพที่ระบุว่าเป็นแบบเจาะลึกพร้อมพยากรณ์โรคในอนาคต ราคาจะอยู่ที่12,000-22,000 บาท และอีกระดับคือ 25,000-29,000 บาท ขณะที่ปี 2556 ที่ผ่านมา การตรวจสุขภาพประจำปีสำหรับผู้ป่วยโรคหัวใจจะอยู่ที่ 5,500 บาท โดยจะตรวจคลื่นไฟฟ้าหัวใจ การแข็งตัวของหลอดเลือด ตรวจหัวใจโดยการวิ่งสายพาน ตรวจวัดความสมบูรณ์ของเม็ดเลือด ระดับน้ำตาลในเลือด การทำงานของตับทั้งหมด การทำงานของไตทั้งหมด ระดับไขมันในเลือดทั้งหมด ระดับยูริคในเลือด ตรวจหาการอักเสบของหลอดเลือด ตรวจปัสสาวะ เอกซเรย์ปอด

       ขณะที่กลุ่มโรงเรียนแพทย์ อย่างศูนย์ตรวจสุขภาพ ศูนย์การแพทย์สมเด็จพระเทพรัตน์ คณะแพทยศาสตร์โรงพยาบาลรามาธิบดี ก็ มีหลายโปรแกรม เช่น โปรแกรมราคา 2,330 บาท จะได้รับการตรวจสุขภาพทั่วไปโดยแพทย์ ตรวจความสมบูรณ์ของเม็ดเลือด ระดับน้ำตาลในเลือด ระดับไขมันในเลือดทั้งหมด การทำงานของตับ AST ALT Total Protein Albumin การทำงานของไต Creatinine ตรวจปัสสาวะ ตรวจอุจจาระ และเอกซเรย์ปอดและหัวใจ หากเป็นโปรแกรมราคา5,890 บาท สำหรับผู้ชาย จะได้รับการตรวจการทำงานของตับและไตทุกอย่าง ระดับกรดยูริก การทำงานของต่อมไทรอยด์ การทำงานของคลื่นหัวใจด้วยคลื่นไฟฟ้า ตรวจอวัยวะในช่องท้องทั้งหมดด้วยคลื่นเสียงความถี่สูงเพิ่มเติม หากเป็นผู้หญิงราคาจะอยู่ที่ 10,340 บาท โดยเพิ่มการตรวจมะเร็งเต้านม อัลตราซาวนด์เต้านม และตรวจภายในและตรวจมะเร็งปากมดลูก เป็นต้น

       สำหรับ รพ.รัฐ อย่างรพ.ราชวิถี สังกัดกรมการแพทย์ กระทรวงสาธารณสุข หากเป็นกลุ่มอายุ 15-35 ปี จะได้รับการตรวจหาความสมบูรณ์ของเม็ดเลือด ราคา 100 บาท ตรวจปัสสาวะ ราคา 80 บาท ตรวจอุจจาระ ราคา 110 บาท เอกซเรย์ปอด 220 บาท และตรวจมะเร็งปากมดลูก โดยทำการตรวจภายในและแปปสเมียร์ 300 บาท ราคาโดยรวมจะอยู่ที่ ประมาณ 800 กว่าบาท สำหรับกลุ่ม 35-45 ปี จะได้รับการตรวจเพิ่มในส่วนของหน้าที่ไต 120 บาท น้ำตาลในเลือด 60บาท ไขมันในเลือด 160 บาท หน้าที่ตับ 180 บาท โรคเก๊าท์ 60บาท ตรวจคลื่นไฟฟ้าหัวใจ 300 บาท ตรวจมะเร็งเต้านมและทรวงอก 1,700 บาท ตรวจมะเร็งต่อมลูกหมาก 300 บาท และตรวจไวรัสตับอักเสบและภูมิคุ้มกัน 310 บาท โดยรวมต้องจ่ายเพิ่มอีกเกือบ 3,200 บาท ก็ตกราคาประมาณ 4,000 บาท ซึ่งบางรายการประชาชนสามารถเลือกได้ว่าต้องการตรวจหรือไม่ ไม่ได้เป็นแบบแพ็กเกจที่กำหนดมาว่าต้องตรวจทั้งหมด

       จากตัวอย่างดังกล่าวจะเห็นได้ชัดเจนว่า การตรวจเหมือนกัน แต่ราคามีความต่างกันพอสมควร โดยเฉพาะการตรวจระหว่าง รพ.รัฐและ รพ.เอกชน ซึ่งแม้จะระบุว่าเป็นการจ่ายเพื่อซื้อบริการที่ดีกว่า และใช้เทคโนโลยีที่ทันสมัยกว่าก็ตาม ซึ่ง น.ส.ทัศนีย์ แน่นอุดร หัว หน้าศูนย์ทดสอบฉลาดซื้อ มูลนิธิเพื่อผู้บริโภค ยืนยันว่า ราคาของ รพ.เอกชน แพงกว่า รพ.รัฐอย่างเห็นได้ชัด ทั้งที่การตรวจไม่ได้ต่างกันเลย แต่ประเด็นที่น่าห่วงคือผู้บริโภคมักถูกแพ็กเกจตรวจสุขภาพต่างๆ ล่อใจให้ไปตรวจ ทั้งที่ไม่มีความจำเป็นต้องตรวจเลย แต่ด้วยความที่เป็นแพ็กเกจเสียเงินแล้วก็ต้องตรวจตามโปรแกรมที่วางไว้ อย่างผู้หญิงที่ไม่เคยมีเพศสัมพันธ์ โอกาสเป็นมะเร็งปากมดลูกย่อมต่ำมาก ก็อาจไม่จำเป็นต้องตรวจ เพราะการรับเชื้อจะมาจากการมีเพศสัมพันธ์

       ในเมื่อการตรวจสุขภาพแบบเหวี่ยงแห และการเห็นโปรแกรมตรวจสุขภาพอะไรที่น่าสนใจก็ตรวจไปเสียทุกอย่าง เพราะวิตกไปก่อนว่าจะเป็นโรค ไม่ใช่ทางเลือกที่ดี แล้วการตรวจสุขภาพในแบบที่เหมาะสมควรเป็นอย่างไร ในมุมมองของผู้บริโภค น.ส.ทัศนีย์ ระบุว่า ต้องรู้จักตัวเองก่อนว่าเราเป็นเพศอะไร อายุเท่าไร มีการใช้ชีวิตประจำวัน หรือพฤติกรรมเสี่ยงอย่างไร จึงค่อยเลือกไปตรวจสุขภาพที่เกี่ยวข้อง เช่น ชอบกินอาหารปิ้งย่าง มีภาวะอ้วน ก็ควรที่จะต้องไปตรวจมะเร็ง เบาหวาน ระดับไขมันต่างๆ ไม่จำเป็นที่จะต้องไปตรวจเสียทุกเรื่อง เพราะไม่คุ้มค่าเมื่อตรวจแล้วก็ไม่พบว่าเป็นอะไร

       “แต่ยังมีคนอีกประเภทที่น่าห่วงคือปรับพฤติกรรมก่อนไปตรวจสุขภาพ เช่น ควบคุมอาหาร ปรับพฤติกรรมก่อนไปตรวจระดับน้ำตาล เพื่อให้ผลเป็นปกติ แต่ภายหลังการตรวจก็กลับมาใช้ชีวิตแบบเดิม ซึ่งหมอก็ยอมรับว่ามีบุคคลประเภทนี้อยู่มาก คือถ้าตรวจสุขภาพแล้วไม่ว่าจะเป็นโรคหรือไม่เป็นโรค หากไม่ปรับเปลี่ยนพฤติกรรมดูแลสุขภาพให้ดีขึ้น การตรวจสุขภาพก็ไม่เป็นประโยชน์”

       สอดคล้องกับ ศ.นพ.สมศักดิ์ โล่ห์เลขา นายก แพทยสภา กล่าวว่า การตรวจสุขภาพที่เหมาะสมต้องดูปัจจัยเสี่ยงของตัวเองเป็นหลัก เช่น มีการกินยาเยอะ ซึ่งอาจมีฤทธิ์ส่งผลต่อตับ และไต ก็ควรไปตรวจตับและไต เป็นต้น และไม่ควรไปตรวจสุขภาพแบบหว่านทั้งหมด เพราะการตรวจสุขภาพไม่ได้รับประกัน 100% ว่าจะไม่เป็นโรคอีกต่อไป เพราะบางอย่างขึ้นอยู่กับพฤติกรรม เช่น เอดส์ เมื่อตรวจแล้วไม่ติดเชื้อ แต่หากมีพฤติกรรมเสี่ยง อีก 2 วันอาจจะติดเชื้อก็ได้ ทั้งนี้ แพ็กเกจตรวจสุขภาพที่อาจบังคับให้ตรวจในสิ่งที่ไม่จำเป็นต้องตรวจนั้น อยากให้แต่ละโรงพยาบาลทำแพ็กเกจให้หลากหลายมากยิ่งขึ้น มีตัวเลือกมากขึ้น เพราะการตรวจสุขภาพบางอย่าง บางคนก็ไม่มีความจำเป็นต้องตรวจ

       ศ.นพ.สมศักดิ์ ยังระบุด้วยว่า สธ.เตรียมที่จะทำมาตรฐานการตรวจสุขภาพ โดยแบ่งตามช่วงอายุว่า อายุเท่าไรควรตรวจเรื่องใด แล้วคนที่มีพฤติกรรมเสี่ยงแบบใดควรตรวจเรื่องใด เพื่อให้ประชาชนทั่วไปไม่ถูกหลอกจากการขายแพ็กเกจ ที่บางครั้งไม่จำเป็นต้องตรวจ โดยจะมีการนำโปรแกรมตรวจคัดกรองที่ HITAP เสนอมาร่วมพิจารณาด้วย

       โดย นพ.ยศ ขยายความว่า โปรแกรมการตรวจคัดกรองที่ HITAP เสนอมี 12 เรื่อง คือ การตรวจเอชไอวี การตรวจโรคตับแข็ง มะเร็งตับ การตรวจโลหิตจาง ภาวะทุพโภชนาการ โรคเบาหวาน โรคหัวใจขาดเลือด โรคหลอดเลือดสมอง โรคมะเร็งปากมดลูก ปัญหาการดื่มสุรา และการตรวจสายตา ซึ่งได้กำหนดช่วงวัยที่เหมาะสมและจำนวนการตรวจไว้ โดยจะช่วยประหยัดงบประมาณ เช่น สิทธิข้าราชการปัจจุบันพบว่าอัตราการตรวจอยู่ที่ 530-1,200 บาท หากตรวจตามโปรแกรมนี้จะเหลืออยู่ที่ 380-400 บาทต่อคนต่อปี และช่วยลดภาวะแทรกซ้อนต่างๆ ลงได้

       ดังนั้น ก่อนที่จะเลือกซื้อโปรแกรมตรวจสุขภาพ โปรดถามตัวเองสักนิดว่าจำเป็นหรือไม่ อย่างไร ตรวจแล้วสบายใจ ได้มาตรฐาน คุ้มค่ากับเงินที่ต้องเสียไปหรือไม่ และที่ขาดไม่ได้คือ ไม่ว่าผลตรวจสุขภาพจะออกมาดีเลิศหรือมีแนวโน้มจะเป็นโรคใดก็ตาม ก็ต้องหมั่นดูแลสุขภาพร่างกายของตัวเองอย่างสม่ำเสมอ
